## When to use a Blameless Post-Mortem

Use it after a service incident when the immediate recovery is complete, a factual timeline is available and the team is ready to learn. It suits engineering managers, technical leads, responders and people responsible for the systems around detection, response and recovery.

## The 60-minute format

### Five Whys: 30 minutes

The team investigates one concrete incident problem through successive input rounds. Each answer must explain the previous one, with contributing factors and uncertainty kept visible.

### Theme Sort: 15 minutes

Participants group causes, contributing factors and action areas into meaningful system themes. Important outliers remain visible rather than being forced into a tidy category.

### Who, What, When: 15 minutes

The team selects the most important themes, using a quick vote if needed, then records who owns each action, what they will do and when it will happen or be reviewed.
