Friday release delay: Five Whys

Five Whys
How Might We
We’re looking at the release that slipped from Friday to Tuesday. Why did we miss Friday? Add the first cause you saw, not a proposed fix.
Mina
Responding to Five Whys
The payment integration failed after we cut the release branch because staging was still using the vendor’s old API.
Jon
Responding to Five Whys
The vendor cutoff was in a Slack thread, but it never made it into the release checklist.
Priya
Responding to Five Whys
QA only got the full build on Thursday afternoon. Before that we could test payments with mocks, not staging.
The failed test is the symptom. All three accounts point to the vendor change entering the release process too late. Before I ask why that dependency stayed in Slack and out of the checklist, has anyone seen a different cause?

When to use it

When the first explanation is probably not the whole story.

Use it for recurring incidents, operational problems, product friction or team-process issues. Bring one specific problem and people who have observed different parts of it.

The method is simple, but good facilitation matters. Each answer must explain the previous one rather than repeat the symptom in slightly more serious language.

  • Each why is collected as a distinct round
  • Alternative cause branches remain visible
  • Restated symptoms are challenged
  • Findings become practical opportunity questions
